If you have medical and/or dental insurance, we will submit your claims for payment as a courtesy. However, in order for our billing department to do this, you must bring in your insurance card(s) at the time of your visit. You must notify our front desk personnel if your insurance coverage changes at any time during the course of your treatment. If we can estimate how much of your bill is not going to be covered by insurance such as co-pays , deductibles, or percentage, we will require this amount to be paid at the time of the visit. PLEASE NOTE: WE CAN ONLY ESTIMATE THE AMOUNT YOUR INSURANCE CARRIER MAY PAY TOWARDS YOUR SERVICES. FINAL DETERMINATION IS MADE BY THE INSURANCE CARRIER AND WE WILL BILL YOU FOR ANY REMAINING BALANCE AFTER THEY HAVE PAID!
We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.
For your convenience, we accept cash, check, Visa, MasterCard, American Express and Discover. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered.
